Alpha-gal – The vaccine connection

Alpha-gal allergy is a rare food allergy triggered by an immune reaction to alpha-gal, a sugar molecule found in most mammals. Unlike many food allergies, symptoms can appear several hours after eating or being exposed to mammalian meat and products, making the condition difficult to recognize. Often associated with tick bites, alpha-gal syndrome can cause reactions ranging from hives and stomach symptoms to severe, potentially life-threatening anaphylaxis.

Risk Factors, Pathogenesis, and Management of Alpha-Gal Syndrome

“Alpha-gal syndrome (AGS) is an IgE-mediated allergy to galactose-α-1,3-galactose (“alpha-gal”), a carbohydrate expressed by non-primate mammals but absent in humans, birds, and fish. Tick bites are the best-supported and most extensively studied cause of sensitization, although the human evidence remains observational. Besides food sources, alpha-gal is also present in bovine- and porcine-derived gelatin used in several human vaccines and in residual mammalian materials introduced during vaccine manufacture. With suspected cases rising roughly 100-fold over the past decade and alpha-gal IgE now detected in 24% of adults in high-burden US states, the immunologic consequences of parenteral alpha-gal exposure warrant investigation.

Conclusion. Dietary exposure to alpha-gal is lifelong and nearly ubiquitous in human populations. Gelatin-containing vaccines represent a near-universal source of parenteral alpha-gal-bearing material and may contribute to alpha-gal syndrome through two non-exclusive pathways: direct sensitization or immune priming followed by tick-bite boosting. Their ability to elicit reactions in alpha-gal-sensitized patients is established and warrants caution in individuals with known AGS. Whether vaccine-derived alpha-gal also contributes to sensitization remains unproven, but convergent human, immunochemical, and animal evidence provides a compelling basis for urgent direct testing. Prospective measurement of alpha-gal-specific antibodies before and after gelatin-containing vaccination, together with direct quantification of alpha-gal in vaccine lots and tick saliva, could resolve the question. In parallel, rigorous tick-bite prevention, prudence in vaccine and drug selection, and controlled trials of mast cell-directed botanicals and auricular acupuncture are needed for a condition presently managed almost entirely by avoidance.”

Gelatin-Containing Vaccines for Varicella, Zoster, Measles, Mumps, and Rubella Induce Basophil Activation in Patients with Alpha-Gal Syndrome – Conclusions: Gelatin-containing vaccines should be administered with caution or avoided in patients with AGS because of their high potential to activate basophils indicating a risk for anaphylaxis.
